Job summary

Pecan Valley Mh-Mr Region in Weatherford, TX seeks a Wraparound Case Manager to provide intensive services for youth ages 3-18 under the YES Waiver program. You will coordinate care, perform assessments, document in the EHR, and collaborate with schools and social services.

Must be self-motivated, able to travel across counties, work with families, and apply trauma-informed care. Bachelor’s in human services, QMHP-CS, and 2+ years of relevant experience preferred; bilingual Spanish/English a

Qualifications

• Bachelor’s degree in a human services field; Master’s preferred.
• QMHP-CS status must be obtainable.
• Minimum two years’ experience with mentally ill youth.
• Strong knowledge of mental health and substance abuse services.
• Must have Texas driver’s license and insurance.
• Bilingual (Spanish/English) is preferred but not required.

Responsibilities

• Maintain understanding of TRR services and UM Guidelines.
• Coordinate Wraparound planning and supervision.
• Complete a uniform assessment to identify needs and strengths.
• Utilize an electronic health record system to document with consumers.
• Meet productivity standards set by the agency.
• Provide community referrals as needed.
• Communicate with referring entities and HHSC staff to ensure fidelity.
• Travel between locations as assigned.
