Job summary

Texas Panhandle Centers is seeking a Children's Behavioral Health Case Manager to deliver trauma-informed care and skills training to youth (ages 3-17) and families across Borger, Perryton, and surrounding areas.

This full-time on-site role emphasizes a multi-disciplinary team approach, with a focus on assessment, treatment planning, and community-based care to improve daily functioning.

Qualifications

• Bachelor's degree from an accredited university in Psychology, Sociology, Social Work, Counseling, or a related Human Services field (required for QMHP credentialing).
• Valid Texas Driver’s License with an insurable driving record.
• At least 18 years of age.
• Employees may be required to utilize their personal mobile phones for two-factor authentication (2FA).

Responsibilities

• Maintain ethical and professional communications using trauma-informed care language.
• Teach and role model skills for youth and families (living, coping, problem-solving, anger management, parenting, self-control).
• Train in Texas Resilience and Recovery approved curriculums.
• Organize culturally competent care activities and collaborate among participants.
• Conduct assessment and treatment planning, provide outreach and education to engage families, and maintain family- and person-centered approaches.
• Provide direct, community-based care (50%+ face-to-face or via phone in home/community settings).
• Submit documentation within 48 hours of service delivery.
• Collaborate with behavioral health teams to promote skill development.
• Adhere to the Business Code of Conduct and report concerns.
