Hill Country MHDD Centers in Kerr County seeks a Yes Waiver Intensive Case Manager/Wraparound Facilitator to engage high-need youth and families in creating, implementing, and monitoring an individualized plan of care. The role requires flexible scheduling, some evenings, and travel.

You will lead monthly Wraparound meetings, develop and evaluate plans, ensure high-fidelity wraparound, and advocate for clients and families while building trusted partnerships within the team.

Formación

• Bachelor's degree in social, behavioral, or human services with QMHP-CS credentialing ability.
• CPR certification and aggression management training within 3 months.
• Knowledge of or experience with Wraparound model and its documentation.

Responsabilidades

• Coordinate and facilitate monthly Wraparound team meetings.
• Develop, implement, and evaluate individualized plans using Wraparound phases.
• Support YES Waiver program implementation and timely documentation.
• Educate families and providers about the Wraparound process.
• Maintain confidentiality and complete required compliance docs.
