Job summary

Pecan Valley Mh-mr Region is seeking a qualified candidate to provide intensive case management through Wraparound facilitation for youth (ages 3-18). This role requires collaboration with schools and social service agencies, travel across the agency’s coverage area, and possible YES Waiver utilization.

Qualifications

• Bachelor’s degree in a human services field from an accredited college or university.
• Master’s Degree is preferred.
• Must obtain QMHP-CS status.
• Minimum of two years of experience with mentally ill youth in a community setting preferred.
• Strong computer operating skills.
• Ability to work independently and in teams with flexible hours.

Responsibilities

• Provide ongoing intensive case management via Wraparound facilitation.
• Coordinate services and travel between agency locations as needed.
• Complete uniform assessments to identify needs and strengths.
• Document in electronic health records with consumer collaboration.
• Participate in monthly staff meetings and data collection activities.
• Communicate with schools, probation and social service agencies to facilitate services.
