Bluebonnet Trails Community Services is seeking a Wraparound Facilitator to coordinate services across systems, support youth and families, and document outcomes in the YES Waiver program. The role emphasizes family-driven, youth-guided planning and strong collaboration with schools and providers.

This position requires a Bachelor’s in Human Services (Master’s preferred) and a Texas driver’s license. Direct service makes up at least 55% of duties, with occasional evening hours to fit family

Qualifications

• Bachelor’s degree in Human Services is required.
• Master’s degree in Human Services is preferred.
• A valid Texas driver’s license is required, with the ability to maintain status.

Responsabilités

• Coordinate services across multiple systems and monitor progress toward outcomes.
• Advocate for youth and families and ensure continuity of care in the YES Waiver program.
• Maintain timely documentation in the electronic health record and manage deadlines.
• Direct service delivery must comprise at least 55% of working hours and may include evening hours.
