Job summary

Paycom - ATS seeks a Wraparound Facilitator to support youth and families within a coordinated care approach in Morgantown, WV. This full-time role emphasizes engaging with families, facilitating plan development, and collaborating with service providers to optimize outcomes.

The candidate should hold a BA/BS in a Human Service field, have 1–2 years of mental health experience, and be capable of crisis response. CPR/First Aid and MANDT certifications are required.

Qualifications

• Engages and communicates effectively with youth and families and team members.
• Collaborates with youth, families, and agencies to secure supports.
• Ability to conduct home/site visits as needed.
• Strong documentation and record-keeping adherence.

Responsibilities

• Engage with youth and families regularly.
• Coordinate wraparound plan meetings and notify team members.
• Complete intake assessments for new clients and assist with walk-in intake.
• Document all activities per policy and maintain EMR notes.
• Attend trainings to ensure fidelity to the wraparound model.
• Provide crisis assessment and response when needed.
• Advocate for youth and families in meetings and hearings.
• Support transition from wraparound to less restrictive care.
