Job summary

Southern Highlands Community Mental Health Center is seeking a Wraparound Care Coordinator in Princeton, WV. This role involves providing individualized care coordination for children and families under the CSED Waiver program, empowering families and ensuring the development of individualized Plans of Care.

The ideal candidate will have a B.A. or B.S. degree in a related field, along with experience in human services, particularly with Wraparound and CSED Waiver services. This position may offer flexible hours.

Qualifications

• Minimum B.A. or B.S. degree in Social Work, Psychology, Human Services, or a related field.
• Prior experience in human services required; experience with Wraparound and CSED Waiver services preferred.
• Must attend all required trainings to maintain and advance knowledge of Wraparound and CSED Waiver services.

Responsibilities

• Build and maintain a Child and Family Team for each youth.
• Initiate contact with families within 3 days of referral.
• Develop and maintain individualized Plans of Care.
• Create a Family Narrative outlining the family’s story.
• Maintain ongoing knowledge of community resources.
