Job summary

FAMILY SUPPORT SERVICES OF SUNCOAST LLC is seeking a Youth Transition and Facilitation Specialist to support the Independent Living team and collaborate with foster parents, CMOs, and community partners to prepare youth for transition from care.

The role coordinates life skills assessments, facilitates transition staffings for 16–17 year olds, links youth to mentoring, and ensures completion of required CFOP measures while guiding post-18 planning.

Qualifications

• Bachelor's degree required.
• 3 years' experience in community engagement or program management.
• Child welfare experience preferred.

Responsibilities

• Coordinate with contracted providers to facilitate and implement Statewide Life Skills Program and related trainings.
• Track life skills assessments and trainings for participation.
• Schedule and facilitate transition staffings for youth aged 16 and 17.
• Connect youth with mentoring providers and opportunities.
• Prepare case information for 17.5-year-old staffings and Extended Foster Care planning.
• Ensure documentation and compliance for CFOP and related requirements.
