Job summary

Community Health of South Florida, Inc. is seeking a Transition to Independence (TIP) Facilitator to coach and support youth aged 14–21 in Miami.

You will conduct needs assessments, develop individualized Transition Support Plans, and coordinate with schools and mental health professionals to ensure services and outcomes. The role requires a Bachelor’s degree in social sciences/rehabilitation/education, valid Florida driver’s license, CPR certification, and the ability to drive youth programs;

Qualifications

• Proficiency in verbal and written communication.
• Must be computer literate.
• Sensitivity to cultural, religious, racial, disability and gender issues.

Responsibilities

• Conduct strength-based needs assessments.
• Coach and support up to 15 youth.
• Collect and record data on progress across goals.
• Facilitate each youth’s Transition Support Plan.
• Collaborate with teachers/guidance counselors for student plans.
• Develop prevention plans for at-risk youth.
• Coordinate with service practitioners to ensure appropriate services.
• Arrange care and supports per the Transition Support Plan.
• Promote parent involvement and engagement.
• Interface with adult mental health case managers as age approaches 18.
• Assist youth in identifying obstacles to independence and education.
• Develop strategies to overcome barriers and collaborate with agencies.
• Build inter-agency networks to improve access to services.
• Refer youth to clinical, medical, and behavioral specialists as needed.
• Document all contacts and services per CHI standards.
• Assist youth in accessing community resources such as employment and housing.
• Manage program resources according to regulations.
• Work flexible hours, including evenings and weekends.
