Job summary

Family Support Services CMO in Largo, FL seeks a Youth Transition and Facilitation Specialist to support the Independent Living team, coordinating with foster parents, CMOs, foster care services, and other providers to ensure teens are prepared for transition.

This role focuses on Life Skills assessments, connecting youth with positive adults, mentoring, and scheduling transitions for youths aged 16–17, while aligning with contract measures and EFC planning.

Qualifications

• Education: Bachelor's degree required.
• Experience: Minimum 3 years in community engagement, program management, or youth services.
• Strong written and verbal communication with attention to detail.
• Ability to work independently and collaborate with multiple stakeholders.

Responsibilities

• Coordinate with contracted providers to implement Statewide Life Skills Program and track assessments.
• Collaborate with community partners to offer teen enrichment programs and evaluate ROI.
• Schedule and facilitate transition staffings for 16–17 year olds and document in FSFN.
• Prepare case information for secondary case management and Extended Foster Care planning.
• Build and maintain mentoring provider relationships and connect youth with mentors.
• Perform other duties as required.
