Job summary

Open Sky Community Services, based in Massachusetts, is seeking Transition Case Managers / Life Coaches to work with youth and young adults to create and carry out transition plans. The role emphasizes trauma-informed, strength-based planning with future-focused goals and coordinated community supports.

The position requires a Bachelor's degree, a valid driver's license, reliability, and the ability to work occasional Saturdays.

Qualifications

• Bachelor's degree required.
• Valid driver's license and acceptable driving record required.
• Reliability, willingness to learn, and desire to promote a better living standard for youth and young adults with mental illness or behavioral challenges.
• Availability to work occasional Saturdays based on program needs.
• Reliable transportation or access to a vehicle to support outreach and other community-based responsibilities.

Responsibilities

• Outreach to engage young adults in the community and connect them with resources.
• Follow and implement TIP model standards and practices.
• Engage young adults with one-on-one, future-focused coaching.
• Provide trauma-informed, strength-based transition planning.
• Connect youth to services and higher levels of care as needed.
