Job summary

Shore Educational Collaborative Chelsea - Student Services in Chelsea, MA is seeking a Community Coach to support secondary students and young adults with disabilities in life after high school. Under supervision of the Director of Transition Services, you will mentor, arrange internships, and assist with job readiness and community participation.

The role focuses on person-centered transition planning, collaboration with schools and employers, and hands-on skill-building for independent living

Qualifications

• Experience supporting students with disabilities in transition planning.
• Ability to organize visits to training and higher education sites.
• Ability to collaborate with teachers, families, and community partners.

Responsibilities

• Support students in exploring personal vision, talents, and career interests.
• Establish internships and employment opportunities with local employers.
• Help students develop resumes and practice soft skills.
