Job summary

Covenant House Alaska is seeking a Success Manager to build meaningful relationships with youth aged 13–24, guide them through housing, health, and educational goals, and manage a caseload of 10–15 individuals experiencing homelessness or trafficking.

The role emphasizes coordinated services, safety planning, and collaboration with multiple agencies to advocate for youth well‑being and long‑term housing stability.

Qualifications

• High school diploma required; bachelor’s in human services or related field preferred.
• Experience with adolescent or disadvantaged populations is preferred; lived experience is a plus.
• Experience with case management or closely related service provision is required.

Responsibilities

• Provide case management in multiple settings and coordinate services with partner agencies.
• Meet minimum case management and documentation standards, including intake and discharge planning.
• Maintain youth success plans and data according to CHA Quality Standards (ETO, AKHMIS, grant systems).
• Complete Coordinated Entry Assessments and ensure information stays current.
• Transport clients as needed using an agency vehicle; ensure safety and appropriate referrals.
