Job summary

North Shore Community Development Coalition (NSCDC) seeks a dedicated Young Adult Housing Case Manager to provide outreach, direct support, and intensive coaching for young adults aged 18-24 experiencing homelessness or aging out of foster care. The role emphasizes a youth-first, housing-first approach and requires building trust and advocating for client stability.

Qualifications

• Excellent interpersonal communication and writing skills.
• Youth-first lens using strengths-based practice.
• Ability to lead and navigate emergency and crisis response efforts.

Responsibilities

• Lead extensive outreach and connect with Young Adults experiencing homelessness or at risk.
• Conduct intake assessments to evaluate needs and establish a plan for support coordination.
• Develop individualized housing plans and connect to service providers and supports.
• Provide follow up and stability services for temporary and long-term housing planning.
• Manage referrals to external service providers (mental health, employment, etc.).
• Coordinate emergency shelter arrangements and placements with local shelters.
