Job summary

Covenant House Alaska is seeking a Success Manager to build meaningful relationships with youth experiencing homelessness or trafficking, guiding them through goal setting and access to housing, health, and education services.

The role manages 10-15 youths ages 13-24, requires strong case management, crisis de-escalation, documentation, and collaboration with community partners to help youth reach permanent housing and self-sufficiency.

Qualifications

• High school diploma required; a Bachelor's degree in human services, social work, psychology, or a related field is highly preferred.
• Experience with adolescent populations; lived experience is an asset; experience in case management is required.
• Knowledge of trauma-informed care and homelessness services; ability to work with trafficking victims and youth.

Responsibilities

• Provide case management in multiple settings and coordinate services with other agencies.
• Meet minimum case management and documentation standards, including intake and discharge planning.
• Have weekly meetings with assigned youth to collaborate on their Success Plans and assist in goal attainment.
• Keep youth success plans and data in CHA data quality standards; ensure assessments are current.
• Complete data entry in CHA data systems (ETO, AKHMIS, grants databases).
• Perform trafficking screenings and safety planning in collaboration with justice and protection systems.
• Engage in collaborative meetings to advocate for youth ( Coordinated Entry, TDMs, etc.).
• Transport clients to appointments using an Agency vehicle, as needed.
• Maintain ethical standards and professional relationships; communicate with leadership and success teams.
