Job summary

New North Citizens’ Council (NNCC) is seeking a HEAL Family Engagement Coordinator to deliver outreach, assessment, and case management for youth ages 16–24 in Springfield, MA. The role connects participants to resources and promotes safety, development, and long-term success.

The position reports to the Director of Youth Support Services, requires a full-time schedule, and offers an hourly wage of $25. Strong collaboration with schools and community partners is essential.

Qualifications

• High school diploma, GED/HiSET, or equivalent.
• Minimum of one year of experience working with adolescents, young adults, or community-based programs.
• Experience or training in youth development, violence prevention, community outreach, human services, case management, or related area.
• Ability to engage youth and work with diverse backgrounds.
• Strong communication, interpersonal, organizational, and documentation skills.
• Ability to collaborate with schools, community organizations, healthcare providers, social service agencies, and other partners.
• Basic proficiency with computers and applications such as Microsoft Office, email, and internet-based systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide youth outreach, prevention education, and case management services to youth ages 16–24.
• Screen and engage youth seeking program services.
• Build positive relationships to support ongoing participation in services.
• Conduct assessments to identify needs, strengths, barriers, and services.
• Provide ongoing case management and support toward individual goals.
• Maintain communication with youth and families to support engagement.
