Job summary

Wraparound Maryland Inc. is seeking a Youth Rehabilitation Service Coordinator to manage a caseload of youths navigating the juvenile justice system.

You will connect them to community resources and develop individualized success plans, coordinating with families and legal entities in New Castle County. Responsibilities include intake assessments, service plan development, progress monitoring, and court or team presentations.

Qualifications

• Bachelor’s degree or related field required.
• 2+ years of experience with at-risk youth, juvenile probation, or community-based social services.
• Must pass intensive criminal background checks and child abuse clearances.

Responsibilities

• Perform intake assessments to determine social, emotional, and educational needs.
• Develop Individualized Service Plans (ISPs) with goals and timelines.
• Monitor progress through regular check-ins and visits.
• Serve as liaison between youth, family, attorneys, probation officers, and school staff.
• Represent youth in court hearings or multidisciplinary meetings.
• Refer youth to substance abuse counseling, anger management, or trauma therapies.
• Provide crisis support during behavioral incidents and create safety plans.
• Maintain confidential electronic records and ensure services meet mandates.
