Job summary

Wisconsin Community Services seeks a dedicated professional to coordinate care for youth and young adults in the CORE program. You will assemble a Youth Team within two weeks of enrollment and develop individualized plans of care, including a 24-hour crisis plan, leveraging community resources.

Responsibilities include monitoring service quality, coordinating transportation, and ensuring compliance with Wraparound Milwaukee policies.

Qualifications

• Bachelor’s degree in a relevant area of education or human services; minimum 1 year mental health service experience.
• Complete 20-40 hours CCS training (DHS 36) and 42 hours mandatory Wraparound training; attend Family Orientation within 6 months of hire.
• Proficiency with computer applications (Windows, MS Word).
• Ability to demonstrate cultural competence with diverse populations.
• Knowledge and skills to work with children, youth, and families.

Responsibilities

• Coordinate care for youth and young adults in the CORE program.
• Assemble a Youth Team within two weeks of enrollment.
• Develop initial and ongoing Plan of Care and a 24-hour Crisis Plan.
• Monitor service quality and liaise for new resources as needed.
• Arrange transportation for youth and coordinate crisis/emergency services as needed.
• Complete required paperwork per Wraparound Milwaukee policies.
• Collaborate with youth, families, and other team members.
• Report emergencies or critical incidents to Wraparound Milwaukee.
