Job summary

Pathfinders Milwaukee is seeking a CCS Care Coordinator to support youth ages 23 and under with mental health or substance use diagnoses. You will coordinate services, develop wellness plans, and connect families with community resources to improve overall quality of life.

Under the supervision of the Director of Comprehensive Community Services, you will assess needs, create treatment plans, document activities in Wraparound Milwaukee systems, and collaborate with partners to ensure timely

Qualifications

• Bachelor's degree in psychology, human services, or education.
• Master's degree preferred; CCS certification preferred.
• Knowledge of Wraparound Milwaukee CCS philosophy and strategies.

Responsibilities

• Coordinate services and act as care facilitator with providers.
• Complete initial assessments and treatment plans per policy.
• Develop wellness plans and hold regular team meetings.
• Document activities and outcomes per standards.
• Ensure timely documentation and billing in the Wraparound system.
• Connect youths to community resources and partners.
• Provide training and consultation to Pathfinders programs.
• Facilitate referrals, transfers, and discharge as needed.
• Support families and natural supports in understanding needs.
• Provide emergency mental health services when required.
• Maintain driver’s license and reliable vehicle for field duties.
