Journey Mental Health Center is seeking a Service Facilitator and Care Coordinator for youth and young adults in the PROPs First Episode Psychosis program in Madison, WI. The role focuses on intake, recovery planning, and coordinating a collaborative treatment team to support participants in the community.

The position requires a Bachelor’s degree in behavioral health, a valid driver’s license, and experience with recovery-oriented practice; Master’s degree preferred and bilingual abilities are

• Bachelor’s degree in a behavioral health or related field.
• Valid driver’s license and access to a vehicle.
• Master’s degree preferred.
• Bilingual abilities with Alta Language Proficiency Test.
• Experience with evidence-based practices in behavioral health.
• Commitment to diversity and inclusion and recovery-oriented care.
• Experience in community-based behavioral health teams.

Mansioni

• Coordinate care for 10-15 youth and young adults experiencing their first episode of psychosis.
• Complete intake process, interviewing consumer and family members to build assessment, recovery plan, and provider team.
• Coordinate Recovery Team Meetings at least every six months to update the recovery plan and goals.
• Engage program participants, family members, and natural supports for collaborative goal planning.
• Maintain regular contact to support engagement and progress.
• Provide or obtain crisis/emergency services as appropriate.
• Monitor service provision and act as liaison for new services/resources.
