Job summary

The Cuyahoga County Juvenile Court is seeking a Placement Aftercare Coordinator (Community Based Treatment Center) to supervise youth placement in CBTC and oversee aftercare planning in the community.

You will coordinate with families, CBTC staff, and multi-system partners to ensure youth receive services, monitor progress, complete required reports, and support successful community reintegration with EPICS-based practices.

Qualifications

• Bachelor’s Degree in Behavioral and/or Social Sciences; three years of youth/adolescent experience.
• Driver's license with valid automobile insurance.
• Ability to work as part of multidisciplinary teams and handle stress.

Responsibilities

• Supervise placement of youth in CBTC and monitor services.
• Coordinate with families, CBTC staff and teammates on treatment progress.
• Prepare reports, court presentations, and case summaries.
• Conduct aftercare planning and referrals to community providers.
• Participate in case staffing and meetings as needed.
