Job summary

Occasional evenings and weekends and travel are required.

Qualifications

• Bachelor’s in Social Work or related human services degree.
• Minimum 2 years’ experience in human services; preferably adoption or foster care.

Responsibilities

• Identify permanent, safe, and loving homes for youth through family and network outreach.
• Collaborate with DSS staff, case managers, therapists, and CHS Permanency team to support goals.
• Conduct diligent searches and engage resources to plan youth futures.
