Job summary

Qualifications

Responsibilities

• Identify permanent, safe, and loving homes for youth through network exploration.
• Conduct interviews, case record reviews, and resource searches to support permanency.
• Collaborate with DSS staff, case managers, and CHS team members.
• Engage resources and planning for youth’s future; maintain documentation and reporting.
