Qualifications

• Bachelor’s Degree in Social Work or Human Service Field from an accredited college or university.
• Must be 21 years of age.
• Must pass a drug screen, MVR, Criminal History and DFPS History Check, and other required background checks.
• Lifting requirements of 30-50 lbs.
• Must have valid driver's license, acceptable driving record and reliable transportation.

Responsibilities

• Coordinates and delivers services and levels of care for youth.
• Completes initial and ongoing assessments by gathering information from children, families, and collaterals.
• Contacts children within 4 hours of referral and coordinates placement.
• Researches and recommends relative placements and facilitates family meetings.
• Develops case plans and permanency goals with children and families, tracking timeframes.
• Determines needed services including mental health, drug/alcohol, respite, and supervised/unsupervised visits.
• Assists families to establish support networks and prepares for moves or reunification.
• Provides intervention during crises and follows court protocols and attire standards.
• Completes court reports, testifies when needed, and ensures court orders are followed.
• Maintains thorough case records and supports kinship/foster placements toward permanency.
• Participates in supervision and team meetings, sharing progress and barriers.
• Supervises and transports children and families as needed and reports incidents per risk management channels.
