Job summary

Saint Francis Ministries seeks a Permanency Specialist to coordinate services for youth across Texas. You'll perform initial assessments, reach out within 4 hours of referrals, and create permanency plans with families to meet goals on a set timeline.

You will lead family meetings, arrange needed services (mental health, addiction treatment, respite, supervised/unsupervised visits), and document thoroughly for court filings. Statewide travel required, strong interagency collaboration essential.

Qualifications

• Bachelor’s Degree in Social Work or Human Service field from an accredited college/university.
• Must be 21 years of age.
• Must pass drug screen, MVR, Criminal History and DFPS checks.

Responsibilities

• Completes initial and ongoing assessments by gathering information from children, families and collaterals.
• Initial contact of children within 4 hours of referral and arranging placement.
• Recommends relative placements and facilitates family meetings.
• Develops and implements case plans and permanency goals with time‑framed targets.
• Determines services needed (mental health, addiction services, respite care, supervised visits).
• Assists families to establish a support network and relationships with kin/community.
• Interacts with the court system, testifies as needed, and ensures court orders are followed.
• Maintains case documentation accurately and timely.
• Supports kinship and foster families to maintain placements toward permanency.
