Job summary

Marshall University School of Pharmacy is seeking a Youth Navigator to provide case management, care coordination, outreach, and navigation services for transition-age youth and young adults (ages 16–26) within the Center of Excellence for Recovery. You will partner with schools, healthcare providers, and social service agencies to ensure coordinated, person-centered care and strong outcomes.

Qualifications

• Bachelor's degree required in Social Work or related field.
• 1-2 years of experience in case management or related fields preferred.
• Experience with transition-age youth (16–26) preferred.

Responsibilities

• Identify and connect eligible youth and young adults through schools and community partners.
• Conduct intake, screening, and eligibility determinations for COE services.
• Develop individualized service and goal plans with education, employment, health, housing and other needs.
• Provide ongoing care coordination through meetings and home visits as appropriate.
• Maintain confidential participant records and outcomes per agency policies.
