Job summary

The New Foster Care in Michigan is seeking a Transition Navigator to partner with youth and families, providing trauma-informed, person-centered case management across five transition domains to support personal, educational, and career goals.

Working under the Program Director and Transition Services Supervisors, you will maintain a caseload, develop individualized plans, coordinate across systems, and travel throughout the Metro Detroit area to meet participants where they are.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Human Services, or a related field.
  • At least 1 year experience with youth involved in foster care, juvenile justice, or other child- and family-serving systems.
  • Familiarity with NASW Code of Ethics.

Responsibilities

  • Maintain a minimum caseload of 20 active participants.
  • Develop individualized plan of service aligned with transition goals.
  • Maintain understanding of goals, barriers, and progress across five domains.
  • Conduct weekly in-person engagements in community, home, or office.
  • Provide in-vivo coaching to strengthen daily living, social, emotional, and problem-solving skills.
  • Transport participants to appointments and support engagement across medical, mental health, court, housing, employment, and education.
  • Identify and access community resources, monitor referrals to completion.
  • Coordinate care across mental health, education, vocational, housing, medical, legal, and community services.
