Diamond Experience Co. is seeking a Qualified Mental Health Professional/Case Manager to provide Targeted Case Management and Rehabilitative Services to youth with serious emotional disturbances and their families.

Responsibilities include collaborating with the clinical team, developing service plans, delivering skill-building in home or community settings, and maintaining accurate EHR documentation.

Qualifikationen

• Bachelor's degree required in social work, mental health, psychology, sociology, or related field.
• CANS/ANSA certification required.
• 2+ years of experience in behavioral health.
• Reliable transportation and valid driver’s license.
• Access to computer and internet.
• Ability to work independently with minimal oversight.
• Strong skills in documentation and using EHR systems.

Aufgaben

• Acknowledge and adhere to Medicaid/Insurance guidelines.
• Develop service/treatment plans focusing on child and family needs.
• Provide skill-building training in home or community settings.
• Offer support and crisis/emergency services via face-to-face, phone, or virtual means.
• Maintain client documentation in the EHR and write progress notes.
• Enter data clearly and timely; complete daily session notes within 24 hours.
• Document non-billable contacts to show follow-up history.
• Assist in tracking output/outcome measures and participate in reviews.
• Engage families and team members to share progress on Service Plan goals.
• Assist clients with daily living tasks to promote independence.
• Collaborate with courts, probation, and community agencies to meet needs.
• Participate in staff meetings and training.
