Job summary

Ozone House in Washtenaw County seeks a Housing Supports Coordinator to provide case management and supportive services for transitional age youth (18-24) facing homelessness or housing instability. You will assess needs, plan services, coordinate housing, health, and education supports, and advocate for clients’ access to resources.

Qualifications

• Bachelor’s degree in social work or equivalent combination of training and experience.
• Experience working with homeless or at-risk youth preferred.
• Strong written and verbal communication skills.

Responsibilities

• Provide case management services to homeless and at-risk youth and families.
• Maintain an active caseload of 15–20 clients.
• Assess needs, determine eligibility, and facilitate housing referrals.
• Develop individualized service plans with youth.
• Provide independent living skills training and budget education.
• Assist clients in obtaining documents (IDs, birth certificates, SSN).
• Help clients access housing, education, employment, and health services.
• Coordinate SNAP, Medicaid, HCV, and other benefits applications.
• Provide transportation and access to technology resources.
• Document progress and maintain case records per funder standards.
