Job summary

Lutheran Social Services of Southern California is seeking a Housing Case Manager to provide supportive services and rental assistance to individuals and families experiencing homelessness under the Rapid Rehousing program.

You will work to remove barriers, coordinate with partners, and help participants achieve housing stability through strength-based, client-centered plans, while maintaining detailed records and reporting.

Qualifications

• Bachelor's degree in social services, human services, counseling, or related field.
• Minimum two years of experience providing case management with vulnerable populations.
• Knowledge of Housing First and Trauma-Informed Practices.
• Experience with HMIS and Coordinated Entry is preferred.
• Proficient in MS Office; tech-savvy.

Responsibilities

• Screen and assess eligibility; conduct intakes for participants.
• Coordinate outreach with CES, 211, and outreach partners.
• Develop goal-driven action plans and monitor progress.
• Assist with housing applications, landlord negotiations, and leases.
• Maintain case files and ensure HIPAA compliance.
