Job summary

Community Partnership of Southeast Missouri (CPSEMO) seeks a dedicated YHDP Housing Navigator to provide trauma-informed, community-based services and housing support for youth up to 24 years old. You will conduct case management, help secure stable housing, and connect clients with essential resources and supportive services.

Responsibilities include building partnerships, maintaining low barriers to access, coordinating transportation, and documenting client progress in HMIS.

Qualifications

• Bachelor’s degree in social work, psychology, public administration, or related field preferred.
• Experience with at‑risk youth and housing instability preferred.
• Strong interpersonal and communication skills with a commitment to advocacy and support for youth.
• Ability to collaborate in diverse, multidisciplinary teams.
• Missouri Driver’s License and reliable transportation with insurance.

Responsibilities

• Provide trauma-informed case management and housing support to youth up to age 24.
• Build and maintain partnerships to enhance service delivery.
• Maintain open communication with supervisors and contribute as a team member.
• Assist clients in securing stable housing and connecting with resources.
• Document client information in HMIS and coordinate transportation when needed.
• Organize outreach events and participate in trainings and meetings.
