Job summary

Community Partnership of Southeast Missouri seeks a Youth Homelessness Demonstration Project (YHDP) Coordinated Entry Specialist. You will serve as the front door for youth housing instability, assess needs, and link eligible youth to housing resources.

The role emphasizes trauma-informed, community-based services, data entry in HMIS, and strong collaboration with partners to place youth into stable housing and coordinate ongoing supports.

Qualifications

• Bachelor’s degree preferred in social work, psychology, public administration, or related field.
• 1+ year in nonprofit social services; management exp preferred.
• Experience with at-risk youth required; lived housing instability preferred.
• Strong communication and collaboration capabilities.
• Valid Missouri driver’s license and willingness to travel.

Responsibilities

• Follow program policies to expedite housing placements for clients.
• Apply Housing First principles with low barriers to entry.
• Maintain a low-barrier access point for youth seeking housing.
• Conduct comprehensive in-person or remote assessments of youth.
• Enter and manage data in HMIS per HUD standards.
• Prepare monthly reports and required documentation.
• Develop partnerships to improve service delivery and referrals.
• Engage in outreach to increase program referrals and awareness.
• Participate in professional development and trainings.
• Collaborate with diverse teams to ensure client outcomes.
• Communicate with supervisor about outcomes and concerns.
• Other duties as assigned.
