Job summary

The Salvation Army Boise Corps is seeking a Youth Homeless Demonstration Program (YHDP) Case Manager to provide intensive, individualized case management for youth and young adults (ages 18–24) at risk of homelessness.

You will partner with participants to create housing stability plans that promote long-term self-sufficiency through stable housing, education, employment, life skills, and community connections.

Qualifications

• Bachelor of Social Work (BSW) and Idaho LSW required; MSW preferred.

Responsibilities

• Maintain an active caseload of youth in YHDP.
• Complete intake assessments, eligibility determinations, and service plans.
• Meet regularly with participants to assess progress and adjust plans.
• Provide housing navigation and tenancy support.
• Coordinate referrals to education, employment, and community resources.
• Document case notes and program activities in WellSky HMIS.
