The Opportunity Alliance is seeking a Case Manager to join the Homeless Youth Services (HYS) program in South Portland, ME. The full-time, 40-hour per week position focuses on intensive case management to help youth and families achieve stable housing and access essential services.

Responsibilities include coordinating care teams, documenting activities, and participating in program initiatives. A Bachelor’s in Social Work or related field is required; LSW preferred.

Formación

• Bachelor’s Degree in Social Work or related field required.
• LSW preferred.
• At least one year of relevant human services experience with youth/families.
• Ability to respond to crisis situations in the moment.
• Ability to work independently and collaboratively.
• Excellent organizational skills and scheduling flexibility.
• Criminal/background checks required.

Responsabilidades

• Provide intensive case management services to youth and/or families in York and Cumberland counties.
• Assist youth and families in finding stable housing and basic needs.
• Link families to services and resources to support stabilization.
• Attend program initiatives and team meetings.
• Coordinate service teams for each youth/family and document promptly.
