Bridge Over Troubled Waters, a Boston-based nonprofit helping runaway and homeless youth, seeks a Rapid Rehousing Case Manager for a full-time role. You will assess housing barriers, plan individualized services, and connect residents to housing, education, and community resources.

The role requires 2–4 years’ experience with youth, knowledge of Housing First, data systems, and advocacy. A driver’s license is required; flexible hours may be needed.

Qualifications

• 2–4 years of experience working with vulnerable youth ages 18–24.
• Bachelor's degree is preferable but not required.
• Experience working with housing programs.
• Knowledge of “Housing First” philosophy and strategies.
• Knowledge and experience with data collection and management systems.
• Knowledge of social service, housing support, and basic benefit systems.
• Valid driver’s license required and continuous clean driving record.

Responsibilities

• Conduct housing needs assessments of Bridge residents to connect them to appropriate services.
• Assess housing barriers and develop Individualized Service Plans to guide participants toward long-term housing and self-sufficiency.
• Provide case management support for a caseload of 30 participants.
• Help residents develop life skills and connect with community resources.
• Maintain client files, logs, and outcome reports for program evaluation.
• Assist youth with locating community health and social services.
• Collect and report program data for internal and funder reporting.
• Ensure respectful and culturally sensitive service delivery.
• Participate in weekly and quarterly program meetings.
