Job summary

Alexander Youth Network is seeking a Case Manager for the On Ramp program to support housing for pregnant and parenting young adults. You will provide direct client services, documentation, and implement Housing First through field and office activities.

Applicants should hold a bachelor’s degree in social work or related field, with 2 years of relevant experience, and strong organizational and communication skills. Fluency in English is required; Spanish is a plus.

Qualifications

• Bachelor’s degree in social work or related field.
• 2 years’ experience with transition-age youth and/or homelessness, mental health or substance abuse.
• Knowledge of Housing First philosophy and strategies.
• Strong organizational skills to meet a demanding workload.
• Detail-oriented with ability to complete required files and maintain contract compliance.

Responsibilities

• Provide direct client services and document accordingly.
• Implement the TIP Model of case management with housing clients.
• Support participants to achieve self-sufficiency and housing permanency.
• Carry out administrative tasks involved in reviewing and maintaining caseloads in office and field settings.
