Pathfinders in Milwaukee is seeking a Supportive Employment Case Manager to assist youth ages 11-25 with resources and targeted case management through a network of partner organizations.

Under the Outreach Drop-In Center Program Manager, you will coordinate services with the Drop-In Center team and community partners, help youth exit streets into housing, and support employment skills development while maintaining records and reports.

Qualificações

• Experience with case management for youth preferred.
• Ability to work with community partners and providers.
• Strong organizational and record-keeping skills.

Responsabilidades

• Coordinate services with Drop-In Center team and partners to maximize youth access to resources.
• Provide intensive case management to help youth exit the streets into housing.
• Increase access to resources in the Greater Milwaukee area through collaboration.
• Assist youth with personal profiles and needs assessments for service access.
• Support employment skills development through experiential learning and soft skills training.
• Maintain client records and complete activity, input, and outcome reports.
• Participate in program and agency meetings and trainings.
• Other duties as assigned.
