Job summary

Prologue, Inc. is seeking a HOP Youth Case Manager to provide intensive case management to homeless youth in Baltimore County. The role focuses on client-centered planning to secure housing and address life needs, with coordination across community services under the Homeless Outreach Program (HOP).

The position reports to the Assistant Director and requires strong communication, teamwork, and a clean driving record to support families and unaccompanied youth up to age 24.

Qualifications

• Bachelor's degree required.
• Experience with special populations preferred.
• Ability to work as part of a team and maintain a good driving record.
• Excellent verbal and written communication skills; ability to manage a caseload.

Responsibilities

• Conduct needs assessments and develop service plans with goals and action steps.
• Meet with clients regularly to track progress.
• Link clients to health, housing, employment, benefits, and education services.
• Maintain accurate case notes and report data in HMIS.
• Participate in monthly case reviews and continuity meetings.
• Engage in community coalitions and outreach activities.
• Assist clients with housing applications and obtaining documentation.
• Utilize company vehicles to connect clients with services and entitlements.
• Ensure fiscal compliance with agency equipment, vehicles and funds.
• Keep client records complete and compliant with standards.
• Complete Relias training and Time/Attendance weekly to support payroll.
• Handle other duties as assigned by supervisor.
