Job summary

The Start Program's Case Manager 2 in Hartford, CT offers full-time work at $22 per hour, with 40 hours weekly on 1st shift (Tue–Fri 10:00 am–6:00 pm; Sat 9:00 am–5:00 pm). Qualifications

• Associate’s Degree required.
• 2+ years relevant experience in direct service and case management field.
• Valid Connecticut Driver’s License.

Responsibilities

• Provide home- and community-based case management to homeless young adults.
• Conduct intake assessments and develop individual service plans.
• Assist youth with housing search and tenancy support.
• Coordinate with treatment providers and community resources.
• Document data entry and manage caseload (8–12 clients).
