Schedule: Full time; 40 hours; 1st shift; Monday-Friday 8 am-4pm.
The Start Program is a nationally recognized program that works with homeless and unstably-housed youth, aged 18-24, located throughout the state of CT through an innovative service delivery model that combines short-term rental assistance with intensive, community-based case management services targeted towards housing stabilization. Through a lens of trauma-informed care and positive youth development, the program partners with young adult participants to identify goals and resources that help them to exit homelessness and achieve long-term stability.
The Case Manager 2 is responsible for providing community-based case management services at the agency-leased shelter to homeless young adults located in Hartford. Responsibilities include;
Working within our Young Adult Services, the Case Manager 2 works with the START program staff to provide services to stabilize and reunite families experiencing complex issues such as substance use, behavioral health matters, and homelessness.
If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
