Job summary

The Connection Inc. is seeking a Case Manager 2 to provide community-based case management for homeless young adults in Hartford, CT.

You will work with START program staff to stabilize families and support housing goals through trauma-informed care and positive youth development. Responsibilities include intake assessments, housing searches, service planning, and coordination with landlords and service providers.

Qualifications

• Associate’s Degree required.
• 2+ years of direct service and case management experience.
• Valid Connecticut Driver’s License required.
• Regular and predictable attendance required.

Responsibilities

• Deliver comprehensive case management services to clients in homes and communities.
• Conduct intake assessments.
• Assist youth in locating available apartments for rent.
• Develop and implement individual service plans to progress toward self-sufficiency.
• Liaise with treatment providers and community resources.
• Provide transportation to clients as needed.

Location: Hartford

Salary: $22.00/hour

Schedule: Full time; 40 hours; 1st shift; Monday-Friday 8 am-4pm.

The Start Program is a nationally recognized program that works with homeless and unstably-housed youth, aged 18-24, located throughout the state of CT through an innovative service delivery model that combines short-term rental assistance with intensive, community-based case management services targeted towards housing stabilization. Through a lens of trauma-informed care and positive youth development, the program partners with young adult participants to identify goals and resources that help them to exit homelessness and achieve long-term stability.

The Case Manager 2 is responsible for providing community-based case management services at the agency-leased shelter to homeless young adults located in Hartford. Responsibilities include;

• delivering comprehensive case management services to clients in their homes and in their communities,
• conducting intake assessments,
• assisting youth in locating available apartments for rent,
• working in partnership with youth to develop and implement individual service plans that will progress them to self-sufficiency,
• liaison with treatment providers and community resources,
• providing transportation to clients as needed,
• developing money management skills with caseload,
• developing employment readiness skills with caseload,
• liaison with landlords and property management staff to support youth's continued tenancy,
• data entry and documentation,
• caseload size will vary between 6-10 young adults at any given time

Working within our Young Adult Services, the Case Manager 2 works with the START program staff to provide services to stabilize and reunite families experiencing complex issues such as substance use, behavioral health matters, and homelessness.

• 2+ years relevant experience in direct service and case management field
• Valid Connecticut Driver’s License
• Regular and predictable attendance is required

Orientation

If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
