Qualifications

• Bachelor’s degree with 1 year experience, or high school diploma with 2 years experience.
• Valid driver’s license required; reliable vehicle in community settings.
• Proficient reading, writing, and oral skills; computer proficiency with electronic record keeping.

Responsibilities

• Establish rapport with clients and maintain professional ethics.
• Enhance daily living skills through coaching and hands-on assistance.
• Develop, implement, and monitor person-centered care plans; manage intake/discharge.
• Coordinate referrals, entitlements, money management, transportation, housing, and wellness services.
• Meet with clients regularly to review progress and appointments.
• Assist with transportation using personal vehicle in community settings.
• Conduct apartment inspections and assist with living space cleanliness.
• Maintain relationships with families and external providers; advocate as needed.
• Facilitate psycho-educational groups and complete program documentation.
