Job summary

Community Healthcare Network (CHN) is seeking a Program Manager for the YACAP program serving youth and young adults aged 13–29. You will oversee HIV prevention activities, PrEP/PEP navigation, testing, and linkage to care, while ensuring data quality, budgeting, and staff supervision.

Under the Director of ID/LGBTQ Programs, you will drive program planning, stakeholder engagement, and compliance with funder requirements to achieve contract deliverables and performance targets.

Qualifications

• Associate’s degree required.
• Bachelor's degree in social work, public health, health education, psychology, human services, or related field preferred.
• Extensive knowledge of HIV/AIDS risk behaviors and related issues.
• Two to four years of HIV program management preferred.

Responsibilities

• Provide overall management and strategic leadership for the YACAP program.
• Develop and implement program work plans, timelines, and deliverables.
• Ensure timely completion and submission of monthly, quarterly, and annual reports.
• Supervise program staff, provide clinical supervision and professional development.
• Coordinate partnerships with community-based organizations and stakeholders.
