Job summary

Community Healthcare Network is seeking a Program Manager for the YACAP program in Brooklyn, NY. You will oversee HIV prevention activities, PrEP navigation, testing, and linkage to care, directing staff and coordinating with medical providers to meet funder requirements.

The role focuses on youth ages 13–29, with emphasis on data-driven program improvements, budget monitoring, and community partnership development to expand reach in high-prevalence areas.

Qualifications

• Extensive knowledge of HIV/AIDS risk behaviors and related issues.
• Two to four years of HIV program management preferred.
• Bachelor's degree in a related field.

Responsibilities

• Provides overall management and strategic leadership for the YACAP program.
• Develops and implements program work plans, timelines, and deliverables.
• Ensures timely submission of reports and required documentation to funders.
• Develops, implements, and monitors program policies and procedures.
• Oversees recruitment, enrollment, and retention strategies for youth 13-29.
• Maintains partnerships with community-based organizations and schools.
• Plans community outreach activities and health education events.
• Supervises program staff and coordinates with multidisciplinary care team.
• Ensures accurate data collection and entry into EMR and data systems.
