SUNY Upstate Pediatrics is seeking a Program Case Manager who will serve as an important member of the Patient Services Team, working with the medical team to provide excellent patient-centered care to address social determinants of health which affect health, functioning and quality of life outcomes, working with patients enrolled in the program with the goal of providing resources and referrals to address health disparities and inequities. Additionally, the Program Case Manager assists with case finding, and supports outreach and program promotion. The primary responsibility of this position is to provide supportive services to patients enrolled in the YACAP AIDS Institute grant-based program. The YACAP Program serves adolescents and young adults seeking sexual health and HIV prevention services.
Bachelor's Degree, or Associates Degree and 2 years experience, Or equivalent combination of education and/or experience required.
Monday - Friday, 8:00 am - 4:30 pm
Salary Range: $44,492, $46,000, DOE
