Job summary

Community Healthcare Network (CHN) seeks a Patient Navigator under the H.I.P.P program to conduct outreach, assist with intake, and educate potential PEP/PrEP and HIV-positive patients seeking services. You will coordinate medical appointments, troubleshoot insurance issues, and enter data into AIRS and CHN’s EMR.

Responsibilities include assessing patient needs, enrolling patients, data entry, coordinating with clinic staff, delivering education on treatment as prevention, and supporting health

Qualifications

• High School diploma or GED preferred with two years’ community work experience.
• Experience in patient navigation and care coordination is helpful.
• Ability to maintain confidentiality and handle sensitive information.

Responsibilities

• Assesses needs of patients seeking PEP/PrEP and related services.
• Assists with intakes, program enrollment, service navigation, and follow up.
• Enter data into AIRS and CHN’s Electronic Medical Record.
• Coordinate medical services with clinic staff and educate patients.
• Support health fairs and trainings; maintain privacy and confidentiality.
