Job summary

Community Healthcare Network (CHN) seeks a Program Manager for the YACAP program serving youth aged 13-29. You will oversee HIV prevention activities, PrEP/PEP navigation, testing, linkage to care, and community outreach with a focus on high-prevalence areas.

Reporting to the Director of ID/LGBTQ Programs, you will supervise staff, monitor budgets, and ensure program compliance with funder requirements while improving service delivery and outcomes.

Qualifications

• Extensive knowledge of HIV/AIDS risk behaviors and related issues.
• Two to four years of HIV program management preferred.
• Experience with youth-focused health services is a plus.

Responsibilities

• Provides overall management and strategic leadership for the YACAP program.
• Develops and implements program work plans and deliverables.
• Coordinates data collection and reporting for funders.

Education

Associate’s degree

Bachelor's degree in social work or related field
