Job summary

Integrated Services for Behavioral Health (ISBH) is a community-minded behavioral health provider serving Southwestern and Central Ohio. We seek a Care Coordinator in Athens, OH to lead care planning and family-team meetings, coordinating services within homes and communities to keep families together and reduce hospitalizations.

The role requires strong communication, HIPAA compliance, and the ability to visit families in various settings.

Qualifications

• Experience coordinating services for children and families in behavioral health is preferred.
• Ability to maintain accurate documentation and meet deadlines across multiple systems.
• Know-how of Medicaid/OhioRISE processes and HIPAA guidelines is preferred.

Responsibilities

• Lead and facilitate child and family team meetings to support youth at home, school, and in the community.
• Provide care coordination in home, community, or preferred location.
• Maintain required documentation to access and maintain OhioRISE benefits.
• Travel within the assigned service area to deliver services and ensure coordination.
