Job summary

Integrated Services for Behavioral Health (ISBH) is a community-minded behavioral health provider serving Southeast and Central Ohio. We seek a Care Coordinator to plan and lead a team for youth in OhioRISE, coordinating services at home, school, and in the community, empowering families with voice and choice.

The role emphasizes collaboration with service providers and families, travel for in-person trainings, and adherence to HIPAA and program standards. McArthur, OH location.

Qualifications

• Experience providing services to children and families in behavioral health settings.
• Hi-Fidelity Wraparound knowledge is preferred (certification provided).
• Two years in a care coordination role with high efficiency and detail.
• Ability to work with autism spectrum disorders and related developmental needs.
• Valid driver’s license and ability to travel within the service area.

Responsibilities

• Develop, lead, and facilitate team meetings for the young person and identified team.
• Provide care coordination within the family’s home, community, or preferred location.
• Collaborate with families, natural supports, and providers to identify needs and services.
• Maintain documentation to access and maintain OhioRISE benefits.
• Attend mandatory trainings and supervision, including travel as required.
• Ensure youth and family voice and choice in all coordinated services.
