Qualifications

• College degree in Social Work, Psychology, Sociology, or related field.
• Three years of full-time experience with youth/young adults (16–29).
• Experience serving low-income, diverse communities and coordinating with partners.
• Proficiency in Google Suite and strong written/oral communication.
• Valid driver's license and ability to pass background check.

Responsibilities

• Conduct intake assessments, evaluate needs, and develop individualized care plans.
• Provide ongoing case management and transition services, linking to housing, health, financial aid, and legal resources.
• Maintain accurate participant files and data in compliance with grant and privacy requirements.
• Coordinate workforce development activities, mock interviews, and job readiness workshops.
