Job summary

Youth Advocate Programs, Inc. is seeking a full-time Case Manager to provide comprehensive case management services to individuals and families in need. You will assess needs, develop individualized service plans, and connect participants with resources and support systems.

The role includes intensive mental health interventions and collaboration with participants to implement service plans and coordinate with appropriate resources and support systems.

Qualifications

• Master’s degree preferred.
• Bachelor’s degree in social work, psychology, or a related field.
• Experience in a community-based agency program.
• Proven experience in case management or similar role.
• Knowledge of social issues and community resources.
• Excellent interpersonal and communication skills.
• Organized and good time management skills.
• Knowledge of EHR systems is a plus.
• Bilingual Spanish is a plus.
• Reliable transportation and valid driver’s license required.
